Parent Consent and Waiver Agreement
1. Program Overview
I understand that the Shining Stars Performing Arts program (“SSPA”) is a performing arts educational program that may include acting, dancing, singing, rehearsals, performances, workshops, and related activities. Participation may also involve travel to performance venues.
2. Assumption of Risk
I understand that participation in performing arts activities involves inherent risks, including but not limited to physical exertion, minor injuries (e.g., sprains or falls), and the potential for more serious injury. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury that may be sustained by my child.
3. Medical Treatment Authorization
In the event of an emergency, I authorize SSPA staff, volunteers, or instructors to seek necessary medical care for my child. I understand that reasonable efforts will be made to contact me prior to taking emergency action. I accept full responsibility for any resulting medical costs.
4. Media Release
I grant permission for SSPA to photograph, video record, or otherwise capture my child’s image or voice during program activities. These may be used in promotional materials, social media, websites, and other platforms.
5. Code of Conduct
I acknowledge that students are expected to behave respectfully and responsibly at all times. Disruptive or harmful behavior may result in disciplinary action, including dismissal from the program. No refund will be given in cases of dismissal due to behavioral issues.
6. Liability Waiver
I release and hold harmless Shining Stars Performing Arts, its directors, staff, volunteers, and affiliated partners from any and all liability, claims, or demands for personal injury, illness, or property damage arising from participation in the program, except in cases of gross negligence or willful misconduct.
7. Refund and Withdrawal Policy
I understand that program fees are 50% refundable up to two (2) weeks prior to the program start date. After this point, no refunds will be issued, including in cases of schedule conflicts, illness, or voluntary withdrawal. All withdrawal requests must be submitted in writing to support@shiningstarsperformingarts.org before the refund deadline.
8. Drop-Off and Pick-Up
I understand that I am responsible for ensuring my child arrives and is picked up on time. Repeated late pickups may result in late fees or removal from the program.
9. Communication
I agree to receive communications from SSPA via email or phone using the contact information provided during registration. I understand it is my responsibility to stay informed about updates, rehearsals, and changes to the program schedule.
10. Consent to Electronic Signature
By checking the agreement box during online registration, I affirm that I have read, understood, and agree to this Parent Consent and Waiver Agreement. I acknowledge that this electronic action constitutes a legal signature.
